All the summarys for AC:bloodlines and even this wikia claim that the game explains the link between Altair and Ezio, hence the title bloodlines. However I have played through Bloodlines and that question is far from answerd. The closest the game actually comes to answering that question is that it implies that Altair and Maria get together and start a family ( confirmed in AC2). but it is beyond me how an Arab man and an English Women spawn a family of Italians. ya ya people migrate but a the end of bloodlines Altair and Maria say they will travel east, and Italy from were they are is west. i know its maybe not that important a detail but its been bothering me for a while so if anyone knows anything about this (maybe something i missed) please comment and give me some insight.
